As part of the after school program, we need all parents to complete the Family Survey for 21st CCLC and LEAP funded Extended Learning Programs. If your child stays for after school, then we need your help. The Family Survey can be accessed on a computer, tablet, or mobile device. The survey will take about 5 minutes to complete. Parents and guardians may choose to complete the survey in English, Spanish, Arabic, or Kurdish.
